The Iraq Campaign Medal is required to be worn with at least 1 star regardless of the number of tours in country. Stars have NOTHING to do with the number of times you were in Iraq. You could have been in Iraq just once and still rate 2 or 3 stars from that one trip. Stars are assigned based on the number of campaigns you were in while there.
